Under section 4 of the Data Protection Acts, 1988 and 2003, you have a right to obtain a copy of any personal information relating to you. To access a copy of personal data held by the Department, in relation to you, please complete the Application Form for Data Access (subject access request) - download word version here. The completed form, along with some photographic identification (passport or drivers licence) should be returned to:

Faoi alt 4 de na hAchtanna um Chosaint Sonraí, 1988 agus 2003, tá sé de cheart agat cóip a fháil d'aon eolas pearsanta a bhaineann leat féin. Chun cóip de shonraí pearsanta maidir leat féin atá i seilbh na Roinne a fháil, iarrtar ort an Fhoirm Iarratais um Rochtain ar Shonraí (iarratas rochtana ón ainmní). Ba cheart an fhoirm chomhlánaithe, in éineacht le cruthúnas céannachta grianghrafach éigin (pas nó ceadúnas tiománaí) a sheoladh chuig:
